If you are planning on trying to do either a Varsity/JV sport or another Varsity/ JV Activity or Sport you will need to fill out and return the completed form to Mr. Rovn the Activities Director.

Schedules for each sport/activity must be attached, along with a summary from the student on how the dual activities will be managed. All information must be duplicated and given to the coaches/advisors, and Athletic Director/Student Activities Director.
I understand that the responsibility for managing both activities and giving early and thorough communications to both advisors/coaches falls to me. I understand that this decision not only affects me but my team members as well. I understand that not being able to manage both activities successfully may mean my role in each activity could be cut back or discontinued at the discretion of the coach or moderator. I agree to the terms set forth.
